PARTICIPANT WAIVER AND RELEASE OF LIABILITY This Waiver and Release of Liability Agreement ("Agreement") is entered into by the undersigned participant ("Participant") and Hoop Dreams Hollywood ("Organization"), and shall be effective upon submission of this form. 1. ASSUMPTION OF RISK The Participant acknowledges that participation in basketball leagues, games, practices, and related activities organized by Hoop Dreams Hollywood involves inherent risks of injury, including but not limited to: sprains, fractures, dislocations, concussions, and other bodily injuries. The Participant voluntarily assumes all such risks, both known and unknown, associated with participation. 2. RELEASE OF LIABILITY In consideration of being permitted to participate in Hoop Dreams Hollywood activities, the Participant, on behalf of themselves and their heirs, personal representatives, and assigns, hereby releases, waives, discharges, and covenants not to sue Hoop Dreams Hollywood, its officers, directors, employees, volunteers, agents, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant while participating in any activities associated with Hoop Dreams Hollywood, whether caused by the negligence of the Released Parties or otherwise. 3. MEDICAL TREATMENT AUTHORIZATION The Participant authorizes the Organization to seek emergency medical treatment on their behalf if deemed necessary. The Participant accepts full financial responsibility for any medical expenses incurred. 4. RULES AND CONDUCT The Participant agrees to abide by all league rules, regulations, and codes of conduct established by Hoop Dreams Hollywood. The Organization reserves the right to suspend or remove any participant for conduct deemed unsafe, unsportsmanlike, or otherwise inappropriate. 5. PHOTO/VIDEO RELEASE The Participant grants Hoop Dreams Hollywood the right to use photographs, video recordings, and other media taken during league activities for promotional, educational, and informational purposes without compensation. 6. INDEMNIFICATION The Participant agrees to indemnify and hold harmless the Released Parties from any claims, losses, liabilities, damages, costs, or expenses (including attorney's fees) arising from the Participant's participation or conduct in connection with Hoop Dreams Hollywood activities. 7. GOVERNING LAW This Agreement shall be governed by the laws of the State of Florida. Any disputes arising under this Agreement shall be resolved in the courts of Broward County, Florida. 8. SEVERABILITY If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. BY SIGNING BELOW (digitally), THE PARTICIPANT ACKNOWLEDGES THAT THEY HAVE READ THIS AGREEMENT, UNDERSTAND ITS CONTENTS, AND AGREE TO BE BOUND BY ITS TERMS.
